Request for Book Donations for Your Literacy Program

Dear Friends of Literacy,

The Literacy Council requires that you complete this form in order to participate in our Book Distribution Program.

Program Requirements:
1. Books donated by The Literacy Council may not be sold under any circumstance.

2. Books donated by The Literacy Council are only for use by programs that promote literacy in the five county Birmingham, Alabama area.

3. Although The Literacy Council occasionally donates books to be used as give-away items, it is strongly suggested that donated books be used as a part of an existing or proposed reading program that has a goal of helping participants a) learn to read, b) improve reading skills for a specific purpose, c) develop an appreciation of reading, etc. For assistance creating a reading program please contact The Literacy Program.

1) Who are the recipients of the books? Please be specific. (e.g. fourth graders at ______ school, summer reading program at ________ church, etc.)
2) Please describe your organization's reading program and how the books will be used to make it more successful.
3) What do you expect to occur as a result of receiving these books? (e.g. improved reading skills, more positive approach to reading, families reading together, etc.) 4) How will you measure the success of your program? 5) Briefly describe the books that you are requesting - fiction, non-fiction, poetry,etc.
